Cox Gymnasium All potential student-athletes must have an updated physical on file (Dated After April 1, 2023) and all electronic athletic paperwork filled out (Click: https://unionnorth-in.finalforms.com) - Click NEW ACCOUNT under the parent icon. Please be sure to have the following items completed prior to the first day of practice. *Student-athlete must have a valid IHSAA athletic physical dated after April 1, 2023 or after. *Student-athlete must have all digital forms completed in Final Forms. *Parents of student-athletes must have all forms completed in Final Forms (Please note when a student is cleared in Final Forms, their name will be highlighted in green) *Student-athlete must be academically eligible to participate - must have passed 5 subjects last semester. *TRANSFER or FOREIGN EXCHANGE students must get clearance from the IHSAA before competing. Please see the Athletic Director to obtain the necessary IHSAA clearance.
